The Standardized Utilization Ratio (SUR), a comparison of device utilization at the national, state and facility level, is a helpful tool healthcare teams can use to prioritize quality improvement efforts toward the prompt removal of central lines when no longer clinically indicated. The current limitation with the SUR metric, however, is it is not all-encompassing. The SUR excludes peripheral IV catheters and any additional central lines a patient may have in excess of one. We know today that peripheral IV catheters, in addition to patients having multiple central lines, put our patients at risk for developing bloodstream infections leading to increased morbidity and mortality. The Vascular Access Density Metric can be used as a more accurate assessment of line utilization. Vascular Access Density is defined as the number of central Line days + peripheral IV line days divided by the number of patient days. Going beyond the SUR and utilizing a metric that measures the use of all invasive lines including peripheral IVs can help healthcare teams better identify areas of high device utilization and target their CLABSI prevention initiatives.
